  1. Good day,
Hope you are doing well, can I visit your hub room 205 for counseling? I am diagnosed 8months ago and taking the medicine almost 9months need medicine.

    1. Hello Neo,

      Apologies, I got confused.

      1. Are you already enrolled to ART?
      2. Do you need an emergency refill or transfer of hub?

Please advise so I can provide assistance.

      Thank you,

      Cj

      1. Any documents needed po? Need to refill, but I am not enrolled yet, as I said I am diagnosed in Canada, binigyan ako ng medication ng 5 to 6 months.

        1. Hello Neo,

I’m really not sure with the legalities.

I think it is best for you to visit our Hub (TPH Cares). Present address is GF, New Wing Building, Tarlac Provincial Hospital. Please nite that Tarlac Provincial Hospital has multiple entrances, just ask guards on duty where is New Wing Building located. Once you enter the said building, the Hub is beside Billing/Payment Room.

For the documents, bring 1 valid ID and confirmatory result – These are the basic requirements for emergency refill.

Please let me know if you have additional questions.

          Thank you,

          Cj

          1. Hello Neo,

Unfortunately, you need to have a confirmatory result which will serve as your prescription prior to our Hub releasing any ART to you.

An alternative for you is to visit our Hub and re-do the testing/enrollment if you don’t have a confirmatory nor no means to obtain a copy of your confirmatory result in Canada.

Not only TPH Cares but all government HIV/AIDS facilities require confirmatory result for any ART request.

I hope you find ways to resolve your problem so you can continue with your medication.

            Best,

            Cj

  2. Good day!

    1. Hello Patient C,
